You authorize JB'Z LOCAL EAT'Z to begin planning your event based on the details above. A deposit may be required to reserve your date, and your date is not confirmed until the deposit is received.

The estimated guest count is used for planning. The final guaranteed count, final total, tax, and any travel or service charges will be confirmed by JB'Z LOCAL EAT'Z. Cancellations must be made in writing and may be subject to the cancellation schedule in your full catering contract.

You agree to notify JB'Z LOCAL EAT'Z in writing of any food allergies or dietary restrictions before the final guest count date. This form does not replace the full Catering Event Contract, which governs the final terms of service.
